How and where to get your wishes

So Erik and focus96 have written some great books on how, where, and why to use your wishes, but where can you get them?

For just less than 1 million gold, you can buy a wish in Gathol B from the Drow merchant.
For about 1.4 million, you can buy a wish from the champions' store if you are level 20.  I wouldn't!
If you are the first to open Beryl Grotto's portal after a reset, you may get a wish.  This seems to happen around 5-10 % of the time.
The same is true for opening the portal to the Plane of the Elderkind.
Apparently you can receive a wish by pulling the rope in the giant's top floor, but I've never had any luck with that.
Nina pointed out tonight that some of the coffins in the crypt can drop wishes.
If you're using a wish for stats, you don't need to buy a wish at all.  Just give the DM your gold in 1 million coin increments.

What about greater wishes?
DDS is well-known since AO drops a greater wish.
The grinning skull guy in UW Coma drops a greater wish.
The boss in Death drops a greater wish.
The boss in the new Crypt area drops a greater wish.
Lord Wick's new quest (read Zedd's book) results in a greater wish.
Finally Murua in the Source drops a greater wish.
